Fashion shows are all about the clothes....right?
What chance is there this might happen in the states?

Well, the show was for a hat designer so why should anyone be distracted by clothes.
The designer gained some attention by playing on people's fears of nudity which is funny when the story you posted blocks all the naughty bits on all the models except the pregnant one.

Actually, no. Fashion week shows are about branding the designer. "The clothes" are only one element in doing that.

It has. Long ago enough that it's now considered cliche and derivative. Which is why you don't see it all that often. Still, there are lots of high end fashion shows in which breasts are visible, at least. They just don't make the mainstream press, in part because it is so common that it is not news, and in part because the mainstream press can't show it in America. You have to go to the shows to see it - or subscribe to one of the services that gives complete pictures of all FW shows.
-Miss Coles is brilliant.
-It is much easier to coordinate one item vs two or more.
-The least amount of distraction, concerning clothing, is the least amount of accessories. This show works very well because there is only one item for you to focus on. If the the show concept is working you should see a naked person wearing a beautiful hat.

I shoot Diane in hats very often. Sometimes it accenuates the outfit, sometimes it does not. I don't think the nudity aspect helped highlight the hats, but it did draw attention to the designer.

Not so sure of that. I have shot a few models old enough to be your mother. (No... Diane isn't one of them).
Well... having a show without clothing got the designer an article and a post in a forum, right?
I think that, in terms of marketing, this was a success. 
It definatly draws attention to the designer. I guess being unclothed would draw less attention away from the hat than clothing would in the long run. Its just a situation people are not use to.
